NOTICE AND AUTHORIZATION TO OBTAIN CRIMINAL BACKGROUND CHECK FOR EMPLOYMENT PURPOSES
As a condition of application for employment and employment by Labor Chaser, Inc. (“Labor Chaser”), I acknowledge and agree that Labor Chaser, or a third party on behalf of Labor Chaser, may obtain a criminal background check about me at any time during my employment with Labor Chaser.
The types of information that may be ordered include but are not limited to: Social Security number verification; criminal, public, educational and, as appropriate, driving records checks; verification of prior employment; reference, licensing and certification checks;
credit reports; and drug testing results.
The results of this criminal background check may include written, oral, or other information about my character, general reputation, personal characteristics and mode of living. The information may be obtained from private and public record sources, including personal interviews of past or present employers, business or personal acquaintances, or others who may have knowledge concerning this information. I understand that Labor Chaser will use the information contained in these reports as a factor in determining my eligibility for employment and, if I become an employee, in determining my continued employment.
Because a criminal background check is considered an “investigative consumer report,” I
understand that Labor Chaser will, upon my written request made within a reasonable period of time after my receipt of this document, make a complete and accurate disclosure to me of the
nature and scope of the investigation requested. I also acknowledge that I have received the accompanying document entitled “A Summary of Your Rights Under the Fair Credit Reporting Act.
